ABSTRACT

Objective: This study aims to investigate the associations between genetic variations of pyroptosis pathway related key genes and adverse events (AEs) of postoperative chemoradiotherapy (CRT) in patients with rectal cancer. Methods: DNA was extracted from the peripheral blood which was collected from 347 patients before CRT. Sequenom MassARRAY was used to detect the genotypes of 43 haplotype-tagging single nucleotide polymorphisms (htSNPs) in eight pyroptosis genes, including absent in melanoma 2 (AIM2), caspase-1 (CASP1), caspase-4(CASP4), caspase-5 (CASP5), caspase-11 (CASP11), gasdermin D (GSDMD), gasdermin E (GSDME) and NLR family pyrin domain containing 3 (NLRP3). The associations between 43 htSNPs and AEs were evaluated by the odd ratios (ORs) and 95% confidence intervals (CIs) by unconditional logistic regression models, adjusted for sex, age, clinical stage, tumor grade, Karnofsky performance status (KPS), surgical procedure, and tumor location. Results: Among the 347 patients with rectal cancer underwent concurrent CRT with capecitabine after surgery, a total of 101(29.1%) occurred grade ≥ 2 leukopenia. rs11226565 (OR=0.41, 95% CI: 0.21-0.79, P=0.008), rs579408(OR=1.54, 95% CI: 1.03-2.29, P=0.034) and rs543923 (OR=0.63, 95% CI: 0.41-0.98, P=0.040) were significantly associated with the occurrence of grade ≥ 2 leukopenia. One hundred and fifty-six (45.0%) had grade ≥ 2 diarrhea, two SNPs were significantly associated with the occurrence of grade ≥ diarrhea, including CASP11 rs10880868 (OR=0.55, 95% CI: 0.33-0.91, P=0.020) and GSDME rs2954558 (OR=1.52, 95% CI: 1.01-2.31, P=0.050). In addition, sixty-six cases (19.0%) developed grade ≥2 dermatitis, three SNPs that significantly associated with the risk of grade ≥2 dermatitis included GSDME rs2237314 (OR=0.36, 95% CI: 0.16-0.83, P=0.017), GSDME rs12540919 (OR=0.52, 95% CI: 0.27-0.99, P=0.045) and NLRP3 rs3806268 (OR=1.51, 95% CI: 1.03-2.22, P=0.037). There was no significant difference in the association between other genetic variations and AEs of rectal cancer patients (all P>0.05). Surgical procedure and tumor location had great impacts on the occurrence of grade ≥2 diarrhea and dermatitis (all P<0.01). Conclusion: The genetic variants of CASP4, CASP11, GSDME and NLRP3 are associated with the occurrence of AEs in patients with rectal cancer who received postoperative CRT, suggesting they may be potential genetic markers in predicting the grade of AEs to achieve individualized treatment of rectal cancer.

ABSTRACT

Chronic pruritus seriously affects the quality of life of patients, which is closely related to stress, anxiety and depression. Prolonged and repeated pruritus can induce negative emotions such as anxiety and depression, while continued increased negative emotions can also promote exacerbation of pruritus, which drives the itch scratch cycle, thereby further aggravating skin damage. More and more studies have explored the mechanism of pruritus, anxiety and depression. This article mainly reviews the clinical relationship between pruritus and anxiety, depression and the new progress of its possible mechanism, providing reference for the prevention, control and effective treatment of chronic pruritus, anxiety and depression.

ABSTRACT

@#Abstract: Objective To summary and analyze the epidemic situation of imported malaria and the prevention and control measures implemented during the new crown pneumonia epidemic in Shenzhen, so as to provide reference for the prevention and control of imported malaria under COVID-19 epidemic. Methods The data on the prevention and control of malaria epidemic in Shenzhen from 2017 to 2021 were collected for analyzing the epidemic situation of malaria, measures taken and the effect of prevention and control with descriptive epidemiological methods. Results From 2017 to 2021, a total of 104 confirmed malaria cases were reported in Shenzhen, all of which were imported from abroad. 97.1% were imported from Africa, mainly falciparum malaria, accounting for 80.8%. The age of onset was mainly young adults, the age group 20-49 years accounted for 81.7%, and most of the patients were overseas migrant workers, accounting for 59.6%. Imported malaria cases were reported in each month, the most was 13 cases in July, then 12 cases in September. From 2017 to 2021, 709 mosquito trap lamps were placed, and 3 523 mosquitoes were captured, with an average mosquito density of 2.60 per lamp and night. Anopheles were not found, and the dominant species were Culex quinquefasciatus. During the outbreak of COVID-19, Shenzhen has implemented a series of measures in terms of improving the working mechanism of multi-departmental cooperation and joint defense, strengthening screening and monitoring, personnel training, mosquito vector control, and material security, so as to achieve early detection, early transfer, early isolation and early treatment. Conclusions A series of measures have been comprehensively implemented to prevent and control imported malaria in Shenzhen while preventing and controlling imported new coronary pneumonia, and achieved positive results. It has realized the timely identification of cases and effective treatment, and prevented the occurrence of severe and fatal malaria cases, as well as halting the spread and spread of malaria outbreaks.

ABSTRACT

OBJECTIVE@#To observe the clinical effect of fire needling on dysphagia due to pseudobulbar paralysis after stroke and to compare the difference in clinical effect between fire needling and swallowing function rehabilitation training.@*METHODS@#A total of 76 patients with dysphagia due to pseudobulbar paralysis after stroke were randomly divided into an observation group and a control group, 38 cases in each group (1 case dropped out in the control group). The both groups were based on conventional western medication treatment. Fire needle pricking was exerted at Lianquan (CV 23), Fengchi (GB 20), Wangu (GB 12), Shuigou (GV 26), Neiguan (PC 6) and Zusanli (ST 36) without needle retaining every other day in the observation group. The control group was treated with the swallowing function rehabilitation training. In both groups, treatment for 2 weeks was as one course and 2 courses of treatment with 2-day interval were required. After treatment, swallowing scores of Fujishima Ichiro and swallow quality of life questionnaire (SWAL-QOL) scores were observed in both groups, and the clinical effect was compared. Follow-up of swallowing scores of Fujishima Ichiro in 4 weeks after treatment was completed to evaluate the clinical effect.@*RESULTS@#The clinical effective rates after treatment and follow-up were 92.1% (35/38) and 94.7% (36/38) in the observation group, higher than 75.7% (28/37) and 83.8% (31/37) in the control group (<0.05). After treatment, the swallowing scores of Fujishima Ichiro and SWAL-QOL scores were increased in the two groups (<0.05), and those in the observation group were higher than the control group (<0.05). The swallowing scores of Fujishima Ichiro were increased during follow-up in the two groups (<0.05).@*CONCLUSION@#Fire needling has a better effect than conventional rehabilitation training in the treatment of dysphagia due to pseudobulbar paralysis after stroke, which can obviously improve the swallowing function and quality of life in patients with dysphagia.

ABSTRACT

Objective: To observe the clinical efficacy of kidney tonifying and essence strengthening method in delaying physiological vascular aging. Method: Sixty-two subjects who completed the study were randomly divided into experimental group (31 cases) and control group (31 cases) with the matching research method. The experimental group was treated with kidney tonifying and essence strengthening recipe orally for 24 weeks, while the control group was not interfered with traditional Chinese medicine (TCM). Score of TCM syndrome in kidney deficiency syndrome, pulse wave velocity, intima-media thickness, plasma homocysteine level and serum superoxide dismutase level were evaluated before and after treatment. Result: Compared with before treatment period, the score of TCM syndrome in kidney deficiency syndrome, pulse wave velocity and plasma homocystenine level decreased, while the serum superoxide dismutase level increased in the experimental group after treatment (PPPPPConclusion: The kidney tonifying and essence strengthening method may delay the aging of physiological blood vessels caused by aging.

ABSTRACT

Objective We analyzed the relationship of-173G/C MIF polymorphisms with soluble MIF in Coronary Atherosclerotic Disease (CAD) patients.Methods A total of 347 patients were selected,of which 229 normal-coronary and 118 Coronary artery disease subjects.Genotyping of-173G/C polymorphisms were performed by PCR and DNA sequencing.Serum MIF levels were measured using an ELISA kit.Patients were classified by coronary angiogram.Results (1) The frequency of the C genotype was higher in CAD patients than in the control.(2) Serum MIF levels was higher in 173C subjects than in 173G subjects.In addition,we found an increase in serum MIF levels in carriers of the (C/C) genotypes the-173 MIF polymorphisms.Conclusion These data suggest that MIF-173G/C polymorphism may be related to the development of CAD in a Chinese population.

[Results] He studied and researched the Shan Han Lun, in combination with his own experience, thought that the ginseng added into the white tiger soup, not only sweating or emesis or purgation, bus also feeble pulse. Should be taken into account the composite factors such as age and physical etc. And must join ginseng in the post partum. Put forward the theory of the Shaoyin of febrile disease. Breaking the postpartum avoid is used for cold. Remove the heat of Yangming to treat lung disease. Treat the febrile disease of pregnancy. He also thought Yangming and feeble pulse should use white tiger and ginseng soup to treat the pulse, then to purge. He was carefully to ask patients'illness. He combined symptoms with pulse condition, comprehensive analysis to both pulse manifestation and symptoms, paid attention to decocting medicine and taking medicine also. [Conclusion] White tiger and ginseng soup mostly treats the cold or warm into the Yangming disease. The basic pathogenesis is the heat in Yangming. Break the cold attack, warm disease from those. To promote the clinical application of white tiger and ginseng soup, through the clinical details in order to improve curative effect, significant contribution to the clinical application of white tiger and ginseng soup.

ABSTRACT

ObjectiveTo compare the therapeutic efficacies between acupoint thread embedding plus Western medication and dry Western medication in treating metabolic syndrome.MethodTotally 320 patients with metabolic syndrome were randomized into a treatment group and a control group, 160 cases in each group. The control group was intervened by ordinary Western medication, while the treatment group was by acupoint thread embedding in addition to the Western medication given to the control group, and Yishu (Extra), Geshu (BL17), Ganshu (BL18), Pishu (BL20), Shenshu (BL23), Tianshu (ST25), Daimai (GB26), and Guanyuan (CV4) were selected as the major points. The therapeutic efficacies, metabolism-related parameters, liver function, and kidney function were compared between the two groups, and the therapeutic efficacies of different syndromes were also compared.ResultThe markedly effective rate and total effectiverate of the treatment group were significantly higher than that of the control group (P0.05), but there were significant inter-group differences in comparing the rest parameters including liver and kidney functions (P<0.05), and the treatment group was superior to the control group. Acupoint thread embedding produced a more significant efficacy in treating metabolic syndrome due to heat in liver-stomach and phlegm-dampness stagnancy compared to the control group (P<0.05), and the efficacy of acupoint thread embedding in treating metabolic syndrome due to phlegm-dampness stagnancy was significantly different from that of the control group (P<0.01).ConclusionAcupoint thread embedding plus Western medication can produce a more significant therapeutic efficacy in treating metabolic syndrome compared to Western medication alone, and it’s safe; of all the patterns, phlegm-dampness stagnancy responds thebest to acupoint thread embedding.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effects of inspiratory muscle training followed by non-invasive positive pressure ventilation in patients with severe chronic obstructive pulmonary disease (COPD).</p><p><b>METHODS</b>This investigator-initiated randomized, controlled trial recruited 88 patients with stable GOLD stage IV COPD, who were randomized into 4 equal groups to continue oxygen therapy (control group) or to receive inspiratory muscle training followed by non-invasive positive pressure ventilation (IMT-NPPV group), inspiratory muscle training only (IMT group), or noninvasive positive pressure ventilation only (NPPV group) for at least 8 weeks. The outcomes of the patients were assessed including the quality of life (SRI scores), maximum inspiratory pressure (MIP), maximum expiratory pressure (MEP), dyspnea (MRC scores), 6-min walking distance (6MWD) and lung function.</p><p><b>RESULTS</b>s Compared to baseline values, SRI scores, 6MWT and MRC scores increased significantly after 8 weeks in IMT-NPPV, IMT and NPPV groups, and the improvements were significantly greater in IMT-NPPV group than in IMT and NPPV groups (P<0.05 for all). In IMT-NPPV and IMT groups, MIP and MEP increased significantly after the training (P<0.05), and the improvement was more prominent in IMT-NPPV group (P<0.05). No significant changes were found in pulmonary functions in the groups after 8 weeks of treatment (P>0.05).</p><p><b>CONCLUSION</b>Inspiratory muscle training followed by non-invasive positive pressure ventilation, compared with inspiratory muscle training or non-invasive positive pressure ventilation alone, can better enhance the quality of life, strengthen the respiratory muscles, improve exercise tolerance and relieve the dyspnea in patients with COPD.</p>

ABSTRACT

Objective:To analyze the salvage effect of transradial emergency percutaneous coronary intervention (PCI)on patients with acute myocardial infarction (AMI).Methods:A total of 100 AMI patients,who hospitalized in our hospital from Feb 2009 to Jun 2013 and received PCI treatment,were randomly selected as subjects.They were divided into transfemoral artery intervention group (TFA group,n=50)and transradial artery intervention group (TRA group,n=50)according to random number method.Door-to-balloon time,operation duration,expo-sure time of X-ray,pressing time of puncture point,length of hospital stay,success rate of operation and incidence rate of complications were compared between two groups.Results:There were no significant difference in operation duration and success rate of operation between two groups (P>0.05);compared with TFA group,there were sig-nificant reductions in pressing time of puncture point [(14.8±5.7)min vs.(3.9±1.6)min]and incidence rate of complications (16.0% vs.4.0%),and significant rise in door-to-balloon time [(18.7±5.6)min vs.(20.1±6.7) min],exposure time of X-ray [(16.7±5.6)min vs.(22.1±6.7)min]in TRA group,P<0.05 or<0.01.Con-clusion:Compared with transfemoral coronary intervention,the pressing time of puncture point of transradial inter-vention therapy and complication are less,so it can be used as preferred therapeutic method for acute myocardial in-farction,and especially more suitable for primary hospitals.

ABSTRACT

Objective: To establish a UPLC-PDA-ELSD method for the fingerprint of Xiaokeqing Granules (XG) and to determine the contents of neomangiferin, mangiferin, timosaponin BII, palmatine, and berberine so as to provide the basis for the quality control of XG. Methods: The chromatographic fingerprints were determined by UPLC-PDA-ELSD using mangiferin and timosaponin BII peaks as the reference peaks, 23 PDA and 10 ELSD common peaks were selected, and the digital quantitative fingerprint of XG was established. Using the third systematic quantified fingerprint method, the quality levels of 28 batches of XG were assessed. The contents of neomangiferin, mangiferin, timosaponin BII, palmatine, and berberine in the 28 batches of samples were determined using external st andard method. Results: Twenty batches of XG were identified as qualified and eight batches were unqualified, in which there were differences in the five main consitituents. The quantitative results were consistent with the fingerprint. Conclusion: The method is accurate, stable, and simple, which could be applied to the quality control of XG.

ABSTRACT

Objective To evaluate the recent clinical efficacy and safety of recombinant human adenovirus-p53 (rAd-p53) combined with radiochemotherapy in treatment for cervical squamocellular cancer (Sqc).Methods 21 patients with cervical squamous cell carcinoma were randomly divided into 2 groups:experiment group (10 patients received treatment with rAd-p53 intratumor injection combined with radiochemotherapy) and control group (12 patients treated with radiochemotherapy alone).The recent efficacy of the two groups were compared at the end of treatment,and major drug toxicity between the two group were compared during the treatment.Results In treatment group,2 cases recieved CR,6 cases PR,2 cases SD,and the rate of total effective was 80.0 %.In control group,3 cases recieved CR,6 cases SD,2 cases PD,and the rate of total effective was 27.3 %.The rate of total effective in treatment group is superior to that in control group (x2 =76.00,P < 0.05).The major reaction of rAd-p53 were transient fever after rAd-p53 administration.The two groups of bone marrow inhibition reaction rate with no statistical difference (x2 =119.50,P > 0.05).Conclusion rAd-p53 combined with radiochemotherapy in treatment of advanced cervical squamocellular carcinoma has a synergistic effect.It is a safe,effective and convenient treatment method for rAd-p53 intratumor injection.

ABSTRACT

<p><b>OBJECTIVE</b>To observe the therapeutic effect of acpuncture treatment for chronic fatigue syndrome (CFS).</p><p><b>METHODS</b>Nighty cases of CFS were randomly divided into an observation group and a control group, 45 cases in each group. The observation group was treated with acupunture at Renying (ST 9), Fengfu (GV 16), Baihui (GV 20); the control group was treated with 250 mL 5% Glucose injectio combined with 20 mL Shenmai injectio. Fatigue Scale (FS) was used to compare the scores between the two groups after treatment.</p><p><b>RESULTS</b>The total scores in the observation group were 9.37 +/- 2.33 and 5.41 +/- 1.96 before and after treatment respectively, and in the control group, they were 9.08 +/- 2.27 and 7.34 +/- 2.03 respectively. FS brainwork integral, physical fatigue integral, and total integral all decreased after treatment in two groups (all P < 0.001), and it decreased much more obviously in the observation group (P < 0.05, P < 0.01).</p><p><b>CONCLUSION</b>Both of the acpuncture treatment and Shenmai injectio are able to decrease fatigue scale score, improve the fatigue symptoms of CFS patients, and the effect of acupucture treatment is obviously superior to that of Shenmai injectio.</p>

ABSTRACT

This study was aimed to investigate the efficacy of moderate intensity regimen, CHG (homoharringtonine, cytarabine and granulocyte colony-stimulating factor (G-CSF)) on the patients with high-risk MDS or MDS-transformed acute myeloid leukemia. 30 newly diagnosed adult patients with high-risk MDS or MDS-transformed AML were enrolled in this clinical trial to evaluate the efficacy of sequential low-dose homoharringtonine/cytarabine chemotherapy combined with G-CSF priming. Homoharringtonine and Ara-C were injected intravenously at doses of 1 mg and 25 mg daily for 14 consecutive days respectively, G-CSF was injected subcutaneously once daily at a dose of 300 microg on 12 hours prior to chemotherapy and continued given until the end of chemotherapy or when the peripheral WBC count reached > 20 x 10(9)/L. This regimen was given only for one course, and followed by conventional chemotherapy as maintenance or consolidation therapy when CR achieved. 33 patients with high- risk MDS and MDS-transformed AML were injected aclarubicin/Ara-C intravenously at doses of 10 mg and 25 mg for 8 and 14 consecutive days respectively, G-CSF was used at the same dose and the same way as the CHG regimen. 33 patients with high-risk MDS and MDS-transformed AML were treated with HHT/Ara-C intravenously at doses of 2 - 3 mg and 100 - 150 mg daily for 7 consecutive days respectively, G-CSF was injected when WBC count was below 4 x 10(9)/L, and it was stopped to be used when WBC count was > 4 x 10(9)/L. The results showed that (1) 14 patients achieved complete remission (CR) (46.67 %) and 7 patients achieved partial remission (PR) (23.33 %) with one course of CHG regimen, total effective rate was 70%; 14 patient achieved CR (42.4%) and 9 patients achieved PR (27.3%) with one course of CAG regimen, total effective rate was 69.7%; 7 patient achieved CR (33.3%) and 3 patients achieved PR (9.1%) with one course of HA regimen, total effective rate was 42.4%. There was no statistical difference between the effective rate of CHG and CAG, but difference was significant between CHG and HA. (2) Agranulocytosis (neutrophil < 0.5 x 10(9)/L) occurred in 12 cases (40%) of CHG-treated patients with a mean 8 days of agranulocytic period, so the infectious complications were less serious and tolerable without treatment-related death. (3) Among 14 out of 30 patients with CR, 9 relapsed, the mean duration from CR to replace was 8.2 months. All relapsed patients reusing CHG regimen did not achieved CR again. (4) Among 13 cases with CR, 6 patients just received HA or DA regimens as consolidatory and intensive chemotherapy after CR have relapsed, the mean CR time was only 6.1 months. Otherwise, the mean CR time of 7 CR patients received alternative succeeded chemotherapy containing mitoxantrone/idarubicin/THP/homoharringtonine/daunorubicin/aclarubicin after CR was 10.6 months; and among them 4 are still in continuous CR. It is concluded that the CHG chemotherapy regimen has a similar effect with CAG but better than HA, and in a saft chemotherapy regimen with slight myelosuppression in clinical application, strong and alternative succeeded chemotherapy is necessary for CR patients to keep longer CR and survival.

ABSTRACT

Comparing to Chitosan, Chitosan-oligosaccharides have several special functions, such as water-soluble, antitumor activity, immunostimulating effects, and antimicrobial activity. The chitosan-oligosaccharide, the molecular weight of which was about 5000, was used as research model. According to the agarose gel electrophoresis and UV spectrophotometer it was proved that electrostatic interaction was playing a very important role in the formation process of chitosan-oligosaccharide/DNA complex. The potential of adsorbing DNA on chitosan-oligosaccharide was analyzed by gel electrophoresis and UV spectrophotometer, and it was indicated that chitosan-oligosaccharide can improve the storage and structure stability of DNA. To check its protection ability to DNA by DNase I digestive experiment, the result showed that chitosan-oligosaccharide could load with plasmid effectively and protect DNA from being digested by DNase I. It was proved that chitosan-oligosacchide was safe and effective for gene delivery and will have a very good future in the field of gene therapy.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the biological difference of clonal cells between myelodysplastic syndromes (MDS) and acute myeloid leukemia (AML).</p><p><b>METHOD</b>Bone marrow (BM) clonal cells (which had cytogenetic markers detected by FISH assay) and blasts were quantitatively analysed in 51 MDS and 11 AML patients. The clonal cell percentage in orthochromatic normoblasts, granulocytes and megakaryocytes were assayed. The biological functions for phagocytosis and oxidation of MDS peripheral blood (PB) neutrophils were compared with that of normal controls.</p><p><b>RESULTS</b>Almost all MDS patients BM had a higher clonal cell percentage (mean 48.2%) than blasts percentage (mean 6.7%) (P < 0.01), but with the subtype of MDS advancing this percentage gap was closing up, and in 11 AML patients no such gap was observed. This gap in MDS patients with + 8 abnormality was smaller than in those with 5q -. In MDS BM, clonal cells were detected in segmented granulocytes (mean 45.9%), orthochromatic normoblasts (mean 46.0%) and mature megakaryocytes (mean 38.0%). In Addition, an approximate amount of clonal cells with the same karyotype abnormality in BM were detected in MDS PB (mean 37.3% in blood vs 48.6% in marrow). Functional analysis showed that the neutrophils in MDS PB could exert nearly normal physiological functions (P > 0.05), but those from AML could not as compared to healthy donors (P < 0.01).</p><p><b>CONCLUSION</b>There is a significant difference in the biological features between MDS and AML clonal cells.</p>

ABSTRACT

To screen NFAT antagonistic drugs and research signal transduction pathway related to NFAT. Four recombinant vectors were constructed. Each consists of three tandem copies of the human IL-2 distal NFAT-AP1 binding site in the context of the minimal IL-2 enhancer, either the sequence from -326 - +46 or the sequence from -89 - +46 (containing only the TATA box), driving a luciferase reporter gene or a destabilized enhanced green fluorescence protein (d2EGFP) reporter gene, respectively. Transient transfection of Jurkat cells was achieved by electroporation with 5 - 10 microg of the above plasmid and one pulse at 200V, 65ms. Plasmid pEFBos-mNFAT1 constitutively expressing murine full length NFAT1 protein was used for transient cotransfection. The results showed that neither of non-stimulation nor PMA or ionomycin stimulation alone could activate the reporter gene except PMA plus ionomycin costimulation. Furthermore, overexpressed murine NFAT1 augmented the activation of either IL-2 promoter or NFAT-AP1 enhancer drived reporter gene compared to the endogenous did. However, the reporter gene expression was nearly completely inhibited by pretreatment for 1h with FK506 at 5 microg/mL and then stimulation for 6-12h with PMA plus ionomycin in the presence of FK506. These findings indicated that such a transient Jurkat cell model offered a potential platform for preliminary screening of FK506 or CsA-like immunosuppressive agents.
